Set up is required for Direct Billing:  After booking your appointment, contact me with your name and  insurance provider.

Please don't include policy or ID numbers in the contact form. I'll send you a secure form after you book your appointment.

Disclaimer:  while the direct billing process is normally reliable and smooth, if your provider rejects all or part of the claim or if for any other reason I am not able to process it, you must pay up front, take an insurance receipt, and contact your insurance company for reimbursement.
