Set up is required for Direct Billing: After booking your appointment, contact me with your name and insurance provider.
Please don't include policy or ID numbers in the contact form. I'll send you a secure form after you book your appointment.

Disclaimer: While the direct billing process is normally reliable and smooth, if your provider rejects all or part of the claim or if for any other reason I am not able to process it, you must pay up front, take an insurance receipt, and contact your insurance company for reimbursement.
This is a free service, and while I do my best to make the process as easy as possible for you, all I can do is submit the claim. I'm a one-person operation, so there is no extra time to troubleshoot if things don't go as expected.
